I only have an online backup at the moment. I used to have an onsite backup (external hard disk), but I got fed up with my backup software. When I upgrade to Windows 8, I'll give File History a try.

For personal data - kinda. I backup the most important stuff to multiple media types. Mostly just pictures/documents. Any data that I deem important gets cloned to my other hard drive, burn DVD's, backup via external HDD (which is stored at work), and store on cloud (if applicable). I need to work on frequency, backing up more often.

I do keep an image of the really important personal stuff at the bank. Business related code and information is stored on a secure server elsewhere. Everything else is just mirror images kept here, inconvenient if lost but not a big deal either. No cloud storage services for me thanks.

No off site backup.

I have an external hard drive which is only connected to my PC for backups, containing a backup of documents, music, a few videos, usually taken once a year or if I make huge changes. This is also written once a year/two yearly to a set of DVDs.
