It looks like it would actually be pretty easy, just making several triangle shapes and filling them with a gradient color.  Then having another layer with a lighter green/yellow gradient at the bottom and change to the darker green at the top.  Do a hard light mix with the top layer so that it would show through.  Just an idea.

DMesh will yield similar triangulation results on a static image which can be touched up in PS afterwards.  It's also free.  Creatogether also sells a brush for Illustrator which will create the same effect.

Thanks guys. Theses tips worked to a degree but didn't produce exactly what I was hoping for, and I don't think I'm creative enough to do this myself.

 

If you know of any plugins or easy tutorials, please let me know.   Thanks.

App for windows which does what you require.... Can't find any decent actions or anything for photoshop, but there's some available for illustrator.
