Need some help with review form

I placed some 3rd party code on one our websites to allow consumers to add a review of our products, etc.

That code though does not work as well as I thought. The owners like a review system they saw on another site and when I looked at the source code it points to the Magento system. A Varien form.

Does a review system (Varien) exist to be able to place on our website? The form is on php based pages but I need for HTML pages.

Any help is appreciated..

How do you want to make a html based review system? It needs some code(php/asp/jsp etc) to connect to the database to save things.
