Import IBM Jazz work items into TFS?

I'm looking to migrate away from Jazz and into TFS, however, I am also trying to minimize the impact on the team (in terms of lost work items or requiring input of old work items again into TFS). Anyone have any experience migrating like this? Any pointers?
