First thing.  "First time settings loaded" should never show that.  It should reload the page automatically, and simply display a message at the top.
Text is kind of hard to read depending on the picture
Settings menu should have the hand cursor when moving over the items
Buttons feel like they don't belong.  Too solid/square for the feel of the website.
Facebook shows solid  always
Clear local storage in the about doesn't fit the button
Should be able to have both the stock bookmarks and custom ones at the same time
Should be able to modify the default bookmarks (ie keep all, but go to inbox instead of gmail).

Hey firey, thanks for the feedback.
Ive ditched the alert asking to refresh the page, now there is a bit of a welcome screen/tutorial. You can check it out by clearing the local storage on the about page and triggering a first run, but if there are settings you don't want to loose, force a refresh (ctrl + f5) and press the "h" key, that will load the welcome screen.
design decisions are up to jason, buttons are still being styled and what not, and i'll probably add the cursor hover in later tonight, but we'll see how busy i am.
Facebook always shows as solid because its the default highlighted bookmark, you can use the arrow keys to move between them and the enter key to select them. Its now explained by the welcome screen :)
As far as both default and custom bmarks, maybe we should have the default bookmarks load into the custom bookmarks, so users can easily do as you said, jsut select the custom bookmarks settings, edit gmail to inbox and save it as it is. And that would also solve the problem of having the default and custom bookmarks at the same time.

Cheers. Simon.

Could be a cool concept, will try to provide some better feedback later but my initial impression was that the alert or notification (that appears on first load) should at least have some opacity otherwise you question if you've come to the right page.
