Electronic Mobile Forms

Can anyone recommend a product for designing and deploying electronic, offline forms that can be filled from both PC/Mobile and stored in SharePoint/SQL?

 

Something like Formotus preferred (stupid expensive), PDF is the current ghetto solution.  Signature support needed.

We use PDF files at work, it's not a "ghetto" solution, we deal in tons of HIPPA compliant data

Another vote on the PDF's we have to use them, (and CMYK)

PDF is ghetto insofar as its still a file based solution, we need something smarter/more DB enabled. 

 

Digitized as opposed to e-sigs are also a point of contention.
