I would say you can put anything for the hostname and I doubt you need a proxy. Everything else is assigned by your provider. Not sure about dsl, but with cable modems you would most likely use dhcp. Just select the Automatic IP (really called DHCP) in that second screenshot you have posted, then hit enter. Otherwise, contact your ISP as what Webgraph has said.
