You use that candle to light the others, ergo the name. You light one candle every night. The first night you light the "shamash" and the first candle. (They are supposed to stay lit throughtout the eight days, however since the candle can't last eight days you just light it again the next day). The second night you light the second candle (as well as the first because it probably burned out) using the shamsh. And so on..... So no you do not extinguish any candle. Here watch the pic, everytime a new candle is lit it represents a new day, note the "shamash" is always lit. And the pic is not incorrect and neither are you. The person lighting the candles is standing behind the menorah, you are just the spectator.
