Brian the dog definately understands Stewie.

The rest of the family gets a general gist of what Stewie wants/is thinking, but I don't think they understand him fluently.

Just my take.

I don't think so, He openly insults his mother in front of her face at one point, and she doesn't notice.

Can they understand the dog then? :p

If they can't understand the baby, how come they let him go out and do stuff? Same as the dog. I swear the dog was out at a dance or something in one episode.

No one understand the baby....beside Brian. So far my concern.

Parents only hear him mumbling like a baby. While Brian the dog hear perfectly why? maybe cause the dog has six sense .....?

And everyone is able to hear Brian talk....cause he is a special dog I presume...

I mean come on..the show is weird itself....

The dog is the one that make more sense that anyone in that family..he is the one that take care of the baby....cause the parents trust him.

So the baby does weird stuff...well yeah but then again so Peter the father.

But here something I just saw on a website. The planet Family Guy.

"The baby of the Griffin's family, Stewie is an unusual baby in that he has developed the abilities to speak, read and think intelligently very fast. According to Stewie the whole World is against him, especially his mother Lois. Stewie is continually trying to kill her, yet all his attempts have failed so far. Although the family can understand him, they ignore his aggressive comments putting them down as silly baby talk. His only friend is his gay teddy bear, Rupert. Stewie himself has shown homosexual tendencies."

They can understand him as much as you can understand any baby. Think about it: Stewie seems to be at the stage where most babies can say some words, but can't really form complete sentences or hold conversations. But you can still figure out what they want most of the time because you can interpret their noises or crying or whatever. There are a lot of instances on the show that go something like...

Stewie: <goes on some angry rant about how stupid someone is or something or how he wants to kill Lois>

Lois: Aww, looks like somebody's cranky, time for a nap!

That tells me that they hear that he is upset, but they think he's just tired, not plotting world domination. The reason Brian can understand him perfectly, I think, is that the show's writers are playing off an old myth that animals and babies understand each other.

They CAN understand Stewie when he actually speaks simply, like a baby normally speaks; the episode of "Kids Say the Darndest Things" was a perfect example of that.

I think they can't understand him, unless its necessary to the plot for them to. If that doesn't make sense remember peter often stops in the middle of a sentace to battle a Giant Chicken.

They can totally understand the stewie, they just choose to ignore certain things.

(paraphrased slightly)

Lois: "Why don't you just eat your broccoli."

Stewie: "Why don't you just BURN IN HELL!"

Lois: "Well! No dessert for you, young man."

Man in airport: "Aren't you a bit young to be flying alone?"

Stewie: "Aren't you a bit old to be wearing braces?"

(Man looks really embarrassed)

Stewie: "Well, I suppose I could give it the old college try. Why don't you put your hands here, you know, to make me more comfortable."

Peter: "Oh-oh, okay."

(Stewie slams the toilet lid on Peter's hands)

Peter: "Yeeeow! Heh, maybe you don't need to pee. Hey, I'll give you a beer - it'll go straight through yah."

Stewie: "Sure, and while we're at it we can light up a doobie and watch porn."

Peter: "Ye-yeah?"

So they understand him, but ignore him cause he is a 3 year old. Or however old he is.
