... I used to think it was always "by accident" and corrected people when they said "by an accident"

...

Lesson learned: Don't start correcting people unless you are sure they are incorrect. ;)

I've noticed some of our pre-schoolers will say "it was by an accident" if they do something "by accident" but have never heard any one over the age of 5 say it :D

Like others have said, it depends on the context, but if something was to happen accidentally then I would say "by accident".

I can't stand it when people insist on using on accident. It infuriates me, but maybe I'm just a grammar nazi, so to speak.
