It's always the same name tagged to your member ID, no matter how many times you change it, unless you change image formats (GIF or JPEG, for example). If you have an image named http://uploads.neowin.net/forum/av-185777.jpg and you upload a new JPEG, it will be named http://uploads.neowin.net/forum/av-185777.jpg also, necessitating a cache clear for you to see the new one.
