Word 2010 Navigation Pane

I am working on a paper and am using a document navigation pane to organize content. The headings are all set as level 2 headings, but there is one heading 'Appendix B' which is not showing up in the navigation pane. I've tried deleting the heading, retyping it, and then reformatting it as a level 2 heading. That didn't work.

Additionally, there is a blank entry in the navigation pane. When I click on it, it jumps to the references page. There is already a references heading in the navigation pane.

How can this be fixed?

I tried uploading the file in the original format, but apparently the forum won't accept the file attachment. I've converted the file to the Word 97-2003 format.

Appendix B Heading 2 text is inside a table. Place it outside.

I don't see any blank entries in the navigation pane. If you're still having issues with that, try showing markers (CTRL SHIFT 8) and going to the blank spot to figure out what's causing it - sounds like a rogue line break got the Heading 2 style.
