Simple Uploader

Does anyone know of a simple uploader?

 

I would like a simple page that has some "browse" bozes so those who aren't tech savvy can upload photos.

Ideally another box for "Folder Name" so people can add their name, so when I'm browsing, I know what photos belong to whom.  I don't need the users to be able to access what is uploaded from the page, but a progress bar would be good.

would be better if you had a login for for each user to log in

I have given it a go and it uploaded an image.

Just got to make a nicer front end and tweak as I want to make it easy for non-tech savvy people to upload photos at an upcoming event.
