Windows 7 with "slate" window border and dark grey wallpaper. Very clean. :)

I haven't use wallpaper since Windows XP because I find myself running programs in full screen most of the time so I don't see the desktop much of the time.

I would think he made a shortcut to the game and changed the icon to that boxart looking one. You can find a bunch of those style icons on deviantART and probably a template on that same site to make your own.
