With stylebuilder, you import the theme, put the PNG in the start button, and recompile it, then use the generated file it as usual :)

Thats probably Yz Shadow. :)

Used iconpackager for the icons, and Styler for the shadow (Y'Z Shadow won't put a shadow on top part of the taskbar)
