New to Vista Customizing... Confused

Ok, so I came from XP where part of the routine ritual after an install was install the uxtheme.dll patch so you could use non-MS themes... it appears that there is no such luck for Vista, so how does one go about installing/using these new themes?

I've double clicked it etc... but it does nothing (I downloaded a theme from the completed themes section). Any help would be appreciated as well as an links you guys could give to any docs concerning customization...
