As Lewism wrote, it's Directory Opus.

It's not the cheapest file manager tho, but i got it with a 25% coupon.

It's Directory Opus as stated earlier.

It makes those covers when you have a folder.jpg file in your album folders

Can't remember where i got the icon from myself :)

Thought it was a Tango icon, but it's not on the tango page.

They use a similar (rotated) icon in the daemon tools bar (IE)
