It's Adium. The contact icons have been set to the largest, the groups have been removed, the list has been shortened to the point where the names get cut off and the opacity of the background has been set to 0.

User icons are set to 48x48 and width of contact list is set to 48x48 so that only the pictures are displayed.

Some users have been given custom user-icons so that I can tell who they are and I deleted people that I don't talk to often (they can still talk to me if they please)

There's so much stuff that can be done in Adium with some playing around and some creativity, I love it!

Just waiting for a Leopard update for it, there's a few niggly things (no show-stoppers by any means) - Just little things like adjusting the size of your display icon and such that I liked in Adium under Tiger doesn't work in Leopard.
