If you're running quake3 in 1024x768 it will have a vertical freq of 60Hz This program allows you to raise that and have a more steady picture DamN : I take it thats the latest version? Everything is in 60Hz and don't forget not everyone uses powerstrip or their refreshfix thingy. Personally I find Powerstrip unworkable, becuase of the many functions that aren't clearly explainded how they work. I use NVmax and Nvidiarefreshratefix.
