Looks like the guy is out of it before this, so when did he get "hit"?  Was it when Mysterio went over th guy's head, and that gave him the whiplash and trachea injury?

Here's when it happened.

It was Mysterio's dropkick that seemed to do it.

 

Nah, it was that he dropped his neck right onto the rope. You can see the dead guy (sorry, don't know his name and don't really care) control himself into the fall, so it was AFTER the dropkick that it happened, and the closeup video takes over right from there, so its when he makes contact with the rope with his neck.

 

As much as I think Mysterio sucks, its not his fault.
